The problem is with ffmpeg/ffmpeg-php installed on your server. You need to reinstall latest version of ffmpeg.

i have the same problem too but only by avi codecs.....

Ive been working on ba's site and have told him that its not installed correctly also. Using putty it says its there but his php info shows its not installed correctly.
