Error uploading video - wrong path?

Logged in as a test user, I tried uploading an 18MB video. The progress bar worked, showed it right up to the end. Then the browser page changed to a 404 page:

Where did this come from? The video didn't get uploaded. This isn't my PATH.

UPDATE: The video DID upload - it is in the /video folder on the server.

However, it does not appear in the Admin Approve Videos nor in the users account.
