Comment Avatar and Profile Avatar not the same, why?

Here's something one of my users discovered....

He tried to update his profile picture by changing it. The Image on his profile page is called 22.jpg. It gets over-written when he uploads a new image but the image that shows up next to the video comments is named 1_22.jpg for some reason... he updated his profile picture, the only thing that got over-written was 22.jpg, therefore the old profile picture called 1_22.jpg never gets over-written and the old image is showing up in the video comments. How do we fix this?
