Got everything to work, the video does upload, the video does convert to flv, shows up and plays, all great!

My only problem is the thumbnail doesn't show up for new uploads. A new thumbnail is made and put in the thumbnail folder, but doesn't show up in the video, just a red 'x'.

Please help! I almost got it working.
