You can check the debug.txt file located in your templates_c directory. More than likely, the problem that you are experiencing is going to be due to a file that ffmpeg is not configured to understand. I would suggest that you change your thumbnail program from ffmpeg-php to mencoder in your admin panel and see if that allows for you to create thumbnails without problems. If so, then you will know that it is a problem with your ffmpeg installation.
