I checked the error log and this is the error that came up:
[Thu Jul 23 07:40:29 2009] [error] [client 141.216.1.4] File does not exist: /home/jjunltd1/public_html/500.shtml
That is second error message, that is after the error, web server try to show the page 500.shtml, and the file is not present in your server. If php script is showing error, it may be permission or ownership problem. Is it cpanel server ?
Try upload a simple php script and see if that works. Ask your host or server admin to check why you get the error.

Yes it is a Cpanel server and I have other PHP scripts uploaded and in use on that server.
And this is both error lines when I try to go to the script

Now when I upload my videos there is no thumbnail, I am using Mplayer and have flvtool enabled
Login to admin area
Site Settings > Miscellaneous > Create Thumbnail with
You have 3 options, select a different option and try upload a new video and see if thumbnails are created.
