Reporting Videos Not Working

So i have had my site running for about a week now and just tested the inappropriate video report button to report a video on my site.

It sent an email notification of the actual report, but when i login to the admin section and goto flagged videos there is nothing there so the reporting is not working. How can i get that sorted?

I also want to add 'duplicate' to the options for reporting, how would i do that?
