Add a custom field to the upload process

I want to add a field in the upload process that will enable affiliates of a content sponsor to enter their affiliate URL. Can someone offer guidance?

i think a selection of custom fields on upload would be very useful. i know other people have asked about this so it might be best to put it in the feature requests forum as im pretty sure its not possible to do without the php files being changed.
